Access and Service Coordination Project
The aim of this project is to demonstrate how schools and youth and community services can work more effectively together to improve the identification, assessment and service provision for young people at risk of disengaging, or who have already disengaged, from education, family and/or the community. The target group for this project is secondary school aged young people.
The project will work with schools and community services to develop an Initial Needs Identification (INI) tool and referral protocols based on the PCP Service Coordination Framework and associated Service Coordination Tool Templates (SCTT). This will involve assessing the appropriateness of the framework and tools for these settings and this population group and modifying them as appropriate. Consultation with key stakeholders is an essential element of the development process.
In addition, the development of an interactive online service directory identifying youth services in both areas for use by young people, parents and carers, schools, and community services in the local area is a key outcome of the project
This project is a partnership between Wyndham City, Hobsons Bay City Council and HealthWest Primary Care Partnership (PCP).
