Note that individuals do not need a medical diagnosis of a mental illness to participate in PHaMs. A person may not be eligible for the service if they are receiving or are entitled to receive non-clinical support through the State Government as a result of detention or incarceration, score under 3 on the Eligibility Screening Tool or are unwilling to address any dual diagnosis / comorbid issues during their participation in the program

Young person must score 3 or more on the Eligibility Screening Tool assessing the impact of mental illness, as wel as complete the FaHCSIA provided data transfer consent form
